What is Clinical Nursing Care?

Clinical Nursing Care is professional nursing support delivered in your home to help you stay safe, well and independent.

Our registered nurses provide personalised assessments, wound care, chronic disease management and ongoing nursing visits, based on your needs. We support people across Port Stephens and surrounding areas.

As part of your ongoing care, our nurses can liaise with your GP and regularly review your support, adjusting assistance levels as your needs change.

Step 1

Call or enquire

Call 02 4981 4404 or send an online enquiry.

Step 2

Tell us what support you need

We’ll listen and ask a few questions to understand your situation.

Step 3

We organise the right support

We’ll arrange the right service for you, and explain funding options if relevant.

Step 4

Care starts at home

Support begins at home based on what you need and what’s appropriate. We’ll confirm the details with you, agree on the next steps, and organise a schedule that suits your routine. If anything changes, you can contact us and we’ll adjust the plan.

What to expect during your first visit?

Your first visit is focused on understanding what you need now and setting up a practical, safe plan.

Before the visit

We’ll confirm your main concerns and the kind of nursing support you’re seeking. If you’re using funding (e.g., Home Care Package / DVA / NDIS), we’ll discuss what information is needed to proceed.

During the visit

A registered nurse will assess your situation and discuss your goals and preferences. If relevant, the nurse may assess wounds, current symptoms, and the supports you already have in place (family, carers, allied health).

After the visit

We’ll agree on what care looks like – what’s included, visit frequency and priorities. Where appropriate, we can liaise with your GP and review care over time to adapt assistance levels as needs change.

After my great uncle became stuck in the red tape of the aged care system, the team at Care For You At Home went above and beyond to support us. They advocated on our behalf with the Aged Care Assessment Team, which ultimately helped his funding come through.
