Starting Support at Home can feel overwhelming, but at Care For You at Home, we make your first visit simple, clear and focused on your needs. The goal is to understand your situation, discuss your preferences and set up a practical, safe plan for ongoing care.
Before the visit
Before we arrive, we’ll speak with you or your carer to confirm your main concerns and the type of support you’re seeking. If you’re using funding – such as a Support at Home Package (formerly a Home Care Package), DVA support, or NDIS plan – we’ll explain what information is needed to proceed, so everything is ready for your visit.
During the visit
A registered nurse will visit your home to assess your needs, discuss your goals and understand your preferences. Depending on your situation, this may include reviewing wounds, current symptoms or the supports you already have in place, such as family, carers or allied health services.
After the visit
We’ll work with you to agree on a personalised care plan. This includes what services will be provided, visit frequency and care priorities. Where appropriate, we can liaise with your GP and review your plan over time to adapt support as your needs change.
Your first visit is all about understanding your needs, giving you confidence in your care, and helping you live safely and independently at home.
